Following documents are to be attached along with Super Distributor Proposal Form "duly stamped and signed by the proposed super distributor"

1. Drug Lisence - 20B and 21B
2. Copy of GST (All Pages)
3. PAN Card
4. AADHAR Card
5. Memorandom of Association/Article of Association/Partnership Deed (Whichever is applicable)
6. Cold Room (Minimum requirement 8 Ft *8 Ft *8 Ft)
7. Manufacturer invoice / Manufacturer drawing of Cold Room mentioning the size of the cold room
8. Sample reading of temperature Data Logger duly signed and stamped by proposed Super distributor and cross verification sign from RSM/Area Incharge
9. Photograph of office , Godown with stacking facility , cold room (inside and outside view) , temperature monitoring device and temperature logger
10. Detail Manpower/Transport facilities available
11. Copy of balance sheet Last 3 years CA certified
12. Purchase Order
13. Distance in KMS from SD to nearest Airport (Google MAP)
14. Annexure A of SD Agreement
